Surgical treatment consists of tenolysis of the hamstring tendon by sectioning the retrotalar pulley. This procedure is sometimes combined with resection of the os trigonum or a large posterolateral talar tubercle that contributes to the impingement. This operation can be performed via open surgery through a 3-4 cm incision behind the medial malleolus, but carries the risk of stretching the calcaneal branch of the great saphenous vein. The arthroscopic approach minimizes this risk. With experience, the structures can be clearly identified, and the pulley section can be performed under good visual control. This is a relatively painless operation that allows for immediate walking without crutches or a brace. Follow-up consists of training tendon gliding and performing the exercises recommended for conservative treatment.
